Mobile IT chargers

I bring your attention to the web address :http://www.dailymail.co.uk/news/article-2403637/Eight-year-olds-iPad-charger-exploded-fathers-hand-giving-huge-electric-shock-killed-her.html.
What is to be done to prevent this and how can we tell, as PAT testers if these items are fused or not. From what I can see, these items are not fused in any way.

I'd be interested to see if the device was indeed "a new genuine Apple charger" as stated in the article, but we'll probably never find out.
